The short version

Why under the tongue

A tablet you swallow has a long journey. It breaks down in your stomach, crosses the wall of the intestine, and then passes through the liver, which destroys a share of the dose before anything reaches the rest of your body. That takes time and the amount of time depends on what you last ate.

The floor of your mouth is thin, and the blood vessels under it sit close to the surface. Medicine dissolving there goes almost directly into your bloodstream. That shortcut is why a 15-minute start is a reasonable expectation here rather than an advertising number, and it is why a heavy dinner matters far less than it would with a swallowed pill.

The route is not novel. Nitroglycerin for chest pain has been given under the tongue for decades, for exactly the same reason.

Timing

Take it around 15 to 30 minutes before you expect to want it. Both sildenafil and tadalafil are in the same tablet, so there is no second dose to think about later — the long component is already covering the back end.

Give the first one an ordinary evening with nothing riding on it. You are learning your own timing, not staging a performance, and checking whether it is working is itself the kind of distraction that suppresses the response.

Food and drink

Food matters much less here than with a swallowed tablet, but a very large or very fatty meal can still slow things a little. One or two drinks are usually fine for most men. Beyond that, alcohol starts working against you: it dulls the arousal signalling the medicine depends on, and it sharpens the headache and light-headedness.

The four mistakes that waste a dose

Swallowing it out of habit. Easily the most common. It still works, just slower and less predictably, and you have thrown away the reason for the format.

Drinking straight afterwards. Rinses away medicine that has not been absorbed yet. Give it a few minutes.

Waiting for it to do something on its own. It will not. It extends your arousal response; it does not create one. Men who sit and monitor themselves usually conclude nothing happened, when in fact nothing was asked of it.

Taking a second because the first felt slow. Never more than one in 24 hours. A second tablet reliably multiplies the side effects and adds very little else.

Storing it

Keep it in the packaging it arrived in, somewhere dry and out of direct heat, and out of reach of children. Do not decant it into a pill organiser — sublingual tablets are more sensitive to moisture than coated ones, and you lose the label that says what it is and what dose.

Never share it. It was prescribed against your health form, not anyone else's, and the contraindications are serious enough that a friend taking one is a genuinely bad idea.

If it does not seem to be working

Give it a fair second attempt first — nerves and technique both affect the first one. If three or four proper attempts are consistently weak, taken sublingually, with real arousal and without heavy drinking, contact the service. Adjusting the strength is a normal part of this, not an admission of anything.
