What it does to your body
An erection is a plumbing job. Arousal releases a messenger that tells the arteries feeding the penis to open. Blood floods in faster than it drains out, pressure builds, and the veins that would carry it away get squeezed shut.
An enzyme called PDE5 clears that messenger away, which is how an erection ends. Sildenafil sits on that enzyme and slows it down. Nothing else changes. The messenger stays around longer, so the arousal you already have travels further than it otherwise would.
The one thing everyone gets wrong
It protects a signal your body makes. It does not make the signal. No arousal means nothing to protect, and nothing to protect means nothing happens. This is behind almost every report of a dose that "did not work".
What the timeline looks like
First 15 minutes. The dose is absorbing under your tongue and levels are climbing. Some men notice warmth in the face toward the end of this.
15 minutes to about an hour. This is where sildenafil is at its most useful. If you are going to get a headache or a blocked nose, it usually turns up here too.
One to four hours. Still working, gradually tailing off. Where sildenafil is prescribed alone, this is the useful window men are told to plan around.
After that. The sildenafil is largely finished. In HardRx this is precisely the point where tadalafil is still going, which is the whole reason both are in one tablet.
Side effects worth knowing about
The common ones are headache, a warm or flushed face, a stuffy or runny nose, mild indigestion, and feeling briefly light-headed on standing. All of them come from the same widening of blood vessels happening elsewhere in the body, and they usually fade over the first few doses.
Less common are back or muscle ache, blurred vision, sensitivity to bright light, or a temporary blue tint to what you see. That last one sounds alarming and is a well-documented, harmless and short-lived effect of this specific drug.
Stop and get help immediately for any of these
Chest pain during or straight after sex. A sudden change or loss of vision or hearing. An erection that has lasted more than four hours. All three are rare and all three are emergencies — call 911 or get to an emergency department rather than waiting.
Who cannot take it
- Anyone using nitrates in any form — tablets, sprays, patches, ointments, or poppers
- Anyone on riociguat or a similar drug
- A heart attack or stroke in the past six months
- Unstable angina, or chest pain brought on by sexual activity
- Blood pressure that is very high or very low and not controlled
- Severe liver or kidney impairment
- Previous sudden vision loss from the eye condition NAION
- Hereditary retinal conditions such as retinitis pigmentosa
What to tell the reviewing doctor
Everything you take, including things you bought off a shelf and anything herbal. Antifungals such as ketoconazole, antibiotics such as clarithromycin, HIV protease inhibitors and grapefruit juice in quantity all change how quickly your body clears sildenafil, which changes the right dose for you. Alpha-blockers for blood pressure or prostate symptoms do not usually rule you out, but they may need the timing spaced.
Leaving something off the form does not improve your chances. It just removes the check the review exists to perform.
Questions about sildenafil
Sildenafil is the medicine inside Viagra. Viagra is Pfizer's brand name for it. HardRx contains sildenafil but is not a Pfizer product and has no link to the company.
That is a common strength for the sildenafil component, but the strength you are prescribed is set by the reviewing doctor from your health form, and the exact figure is printed on your label.
Sildenafil on its own is largely finished within four to six hours. In HardRx the tadalafil component keeps going well beyond that.
Far less than with a swallowed tablet, because dissolving under the tongue skips digestion. A very heavy meal can still slow things a little.
A drink or two is usually fine. Heavier drinking dulls the arousal signal the medicine depends on and makes the headache noticeably worse.
